Payment for medical services is the patient’s responsibility. We require payment in credit card for services at the time services are performed.

At this time, we only accept payments in credit card. Cash or checks will not be allowed. Please plan for your appointments accordingly.

IMPORTANT INSURANCE INFORMATION


Contracted Insurance Plans (HMO, PPO, etc.)

If our office has a contract with your insurance provider, we file all claims with them. You are responsible for the payment of estimated co-insurance or co-pay due, or deductible amounts at the time of service. It is your responsibility to obtain required authorizations from your insurance or primary care physician ahead of any appointments.

Failure to have a current authorization can result in rescheduling your appointment or full payment for all services related to your appointment.

Non-Contracted Insurance Plans (Indemnity Insurance)

We ask that you pay estimated co-pay, deductible, co-insurance, or non-covered services at the time of your appointment. Filing your claim does not take the place of your responsibility to pay for services received.

It is important to recognize that your insurance policy is an agreement between you and your insurance company.

Benefit assignments do not take the place of your responsibility to pay for services received. Capital Endocrine & Diabetes is not responsible for determining or confirming insurance benefits.

Verification of benefits is not a guarantee of payment by your insurance company. Final determination is made by your insurance company at the time the claim is received. We do not coordinate benefits with secondary insurance carriers.
